Alabama Medicaid will relocate its Birmingham district office on Wednesday.

The new office will open tomorrow at the Beacon Ridge Tower, 600 Beacon Parkway West, Suite 105, in Homewood.

The current location at 468 Palisades Blvd. in Birmingham will operate through the end of today, with staff remaining there accepting completed applications through the end of the day.

Recipients may apply for Medicaid online or by mailing signed applications to P.O. Box 304839, Montgomery, AL 36130-4839. Current recipients needing assistance are encouraged to contact their caseworkers by phone during the transition.

Admission To The Program

There is not an age requirement for this program, however a recipient must be financially eligible for Medicaid and meet the programs level-of-care criteria. Once accepted, a case manager will work with the client to develop a care plan based on the clients needs and choices. Services available through the Medicaid Waiver program, include: case management, personal care, homemaker, companion services, respite care, adult day health or nutrition/frozen meals.
