North Carolina Health Choice offers health coverage for children whose families make too much money to qualify for Medicaid, but do not make enough money to afford private or employer-sponsored health insurance. Ccnc/ca Service Referrals & Care Coordination

Effective with dates of service Nov. 1, 2016, CCNC providers shall NOT enter a National Provider Identifier as the CCNC payment authorization number for claims processing.

Providers should not receive claims denials due to CCNC payment authorization for dates of service after Oct. 31, 2016.

Service referral requirements are NOT changing for CCNC providers

Providers are encouraged to continue supporting the CCNC program which is designed to improve quality and access to care manage appropriate utilization of services and achieve cost-effectiveness through care coordination with CCNC PCPs.

CCNC PCPs must continue to adhere to care coordination practices as set forth in Section IV of the NC DHHS Agreement for Participation as a CCNC Provider. The PCP in the medical home will continue to arrange service referrals for medically necessary health care services and specialty care as required by the Agreement.

If you do not have any of this information you may still complete an application and interview for services. How to Apply for Medicaid in North Carolina and What is Covered

North Carolina Medicaid is concerned about your health and wellbeing during the COVID-19 outbreak. In response to COVID-19, we have made temporary changes in how you get Medicaid services. We are also providing new ways for health care providers to care for you.

  • Your benefits will not end or need to be reviewed during this emergency. All Medicaid and NC Health Choice eligibility will be extended for the duration of the COVID-19 crisis.
  • You can get up a 90-day supply of most prescriptions. You can also get all your prescriptions filled at the same time to reduce trips outside yoru house.
  • You can access primary care, specialists, mental health, and some developmental disability services via telehealth. Telehealth is a way your medical providers can provide you care by telephone, smartphone, or computer.

Unitedhealthcare Dual Complete Plans

Plans are insured through UnitedHealthcare Insurance Company or one of its affiliated companies, a Medicare Advantage organization with a Medicare contract and a contract with the State Medicaid Program. Enrollment in the plan depends on the plans contract renewal with Medicare. This plan is available to anyone who has both Medical Assistance from the State and Medicare. Tailored Plans Will Start December 1 2022

On December 1, 2022, NC Medicaid will have a new type of health plan. It is called the Behavioral Health and Intellectual/Developmental Disabilities Tailored Plan . People who need certain services to address a mental health disorder, substance use disorder, intellectual/developmental disability or traumatic brain injury may qualify for the Tailored Plan that serves their county.To learn more, go to Tailored Plan services.

Primary Care Providers Partnering With Ccnc Networks

Managed Care

Once enrolled as a Carolina Access PCP, providers may inquire on serving as a PCP in the NC Medicaids enhanced PCCM program known as CCNC. To participate, providers are required to sign a separate contract with the local CCNC network. Carolina Access PCPs who wish to participate in the CCNC program should contact the CCNC Network serving their county.

PCPs participating in the CCNC network are paid on a fee for service basis, plus an enhanced per member per month care management for each assigned beneficiary. The enhanced fees are noted as follows:

Unitedhealthcare Senior Care Options Plan

UnitedHealthcare SCO is a Coordinated Care plan with a Medicare contract and a contract with the Commonwealth of Massachusetts Medicaid program. Enrollment in the plan depends on the plans contract renewal with Medicare. This plan is a voluntary program that is available to anyone 65 and older who qualifies for MassHealth Standard and Original Medicare. If you have MassHealth Standard, but you do not qualify for Original Medicare, you may still be eligible to enroll in our MassHealth Senior Care Option plan and receive all of your MassHealth benefits through our SCO program.

Family And Childrens Medicaid

  • Covers children through age 20, parents and caretakers of minor children under the age of 18, and pregnant women who meet income requirements.
  • Family Planning Program is designed to reduce unintended pregnancies and improve the well-being of children and families. Services are provided to eligible persons of any age who are ineligible for all other Medicaid programs.
  • North Carolina Health Choice Health Insurance for Children provides reduced price comprehensive coverage for children six to 18 years whose family income meets certain limits but is too high to qualify for Medicaid.

Food And Nutrition Services

The Food and Nutrition Program is a federal food assistance program that provides low-income families a monthly supplemental allotment of benefits issues via Electronic Benefit Transfer cards . The Food stamp program is an entitlement program, so all eligible individual and households can receive assistance.

You may be eligible for Food and Nutrition Services if your total income falls below the appropriate gross income limits for your household size. Eligibility workers determine which income limit applies to your household.

Eligible individuals will receive Food and Nutrition benefits no later than thirty days from the date of application. Individuals with special circumstances will be given an opportunity to receive Food and Nutrition benefits within seven days of the application. Enrolling Nc Medicaid Beneficiaries In Ccnc

Medicaid beneficiaries are educated and enrolled in CCNC by the local Department of Social Services office.

Beneficiaries have freedom of choice when selecting his/her preferred primary care provider. If a beneficiary is enrolled with another provider but wants to join your CCNC/CA practice, ask them to fill out a CCNC/CA Enrollment Form for Medicaid Recipients. Mail or fax the completed form to the DSS in the county in which the beneficiary resides.

If the beneficiary prefers to choose another CCNC provider, refer them to their caseworker at the local DSS office.

Icipation Requirements Of New Primary Care Providers

Medical providers interested in serving as a NC Medicaid Carolina Access PCP must complete an online application accessing NCTracks. NCTracks Provider Enrollment

Applicants must meet all program requirements and qualifications for which they are seeking enrollment before they can be enrolled as an NC Medicaid provider. Provider Permission Matrix.

When completing an Individual or Organization Provider Enrollment application, you will be given the option to also enroll as a PCP in the Carolina Access program, if your provider type qualifies you to participate. CCNC/CA Eligible Provider Types

An eligible Carolina Access Provider must:

  • Perform primary care services that include certain preventive & ancillary services
  • Create and maintain a patient/doctor relationship
  • Provide direct patient care a minimum of 30 office hours per week
  • Provide access to medical advice and services 24 hours per day, seven days per week
  • Refer to other providers when the service cannot be provided by the PCP
  • Provide oral interpretation for all non-English proficient beneficiaries at no cost
