Medicaid Is Jointly Financed By States And The Federal Government

How do I qualify for Medicaid Financially?

Medicaid is financed jointly by the federal government and states. The federal government matches state Medicaid spending. The federal match rate varies by state based on a federal formula and ranges from a minimum of 50% to nearly 75% in the poorest state. Under the ACA, the federal match rate for adults newly eligible was 100% for 2014-2016, phasing down gradually to 90% in 2020 and thereafter . The federal matching structure provides states with resources for coverage of their low-income residents and also permits state Medicaid programs to respond to demographic and economic shifts, changing coverage needs, technological innovations, public health emergencies such as the opioid addiction crisis, and disasters and other events beyond states control. The guaranteed availability of federal Medicaid matching funds eases budgetary pressures on states during recessionary periods when enrollment rises. Federal matching rates do not automatically adjust to economic shifts but Congress has twice raised them temporarily during downturns to strengthen support for states.

Total federal and state Medicaid spending was $577 billion in FY 2017. Medicaid is the third-largest domestic program in the federal budget, after Social Security and Medicare, accounting for 9.5% of federal spending in FY 2017. In 2017, Medicaid was the second-largest item in state budgets, after elementary and secondary education .

    How Does Medicaid Work

The Medicaid program does not provide health care services for everyone. Your application will be reviewed for all Medicaid programs. If you are found eligible for Medicaid, you will get:

    • A letter in the mail from Medicaid telling you about your coverage
    • A Medicaid ID card
    • A Healthy Louisiana plan ID card, if you enroll in a plan.

Each time you go to your health care provider or pharmacy, take your Medicaid ID and Healthy LA plan ID cards with you. Show them at all medical visits before you receive any service. The health care provider or pharmacy will tell you if they accept your Medicaid benefits. You should also ask your health care provider or pharmacy if the service or prescription is covered by Medicaid before receiving the service or filling a prescription.

Medicaid does not pay money to you instead, it sends payments directly to your health care provider or pharmacy.

Medicare Can Pull You Out Of The Coverage Gap

Although the transition from expanded Medicaid to Medicare can be financially challenging, eligibility for Medicare will likely come as a welcome relief if youve been in the coverage gap in one of the 11 states that have refused to expand Medicaid.

Being in the coverage gap means you have an income below the federal poverty level. Once you transition to Medicare so as long as your assets dont disqualify you youll be eligible for the most robust MSP, which covers Original Medicares premiums, deductibles, and coinsurance.

Even if your assets do make you ineligible for an MSP, you could still enroll in Original Medicare or a zero-premium Medicare Advantage plan and pay just $148.50/month in premiums .

And although its not an ideal solution, you do have the option to reject Part B and just enroll in Part A, which is premium-free as long as you or your spouse have worked at least 10 years in the U.S. and paid Medicare taxes.

Although the asset tests in most states might still put an MSP or full Medicaid coverage out of reach once you transition to Medicare, the option to have health coverage for under $160/month is far better than the alternatives that exist for most people under age 65 in the coverage gap.

In most states, unsubsidized premiums for ACA-compliant coverage are three times a high for a 64-year-old versus a 21-year-old, and people in the coverage gap are not eligible for any financial assistance with their coverage.

Which States Allow Qualifying Income Trusts

Its important to note that not all states allow the use of QITs to qualify for Medicaid. Some states allow what is called a Medicaid Income Spend-Down. This allows a senior to qualify for Medicaid by spending down excess monthly income on medical care. . Once a senior has spent down their income to the qualifying limit, one is eligible for Medicaid.

However, other states are known as income cap states. These states have a hard limit on the amount of income that is allowed for Medicaid eligibility. And in these states, Medicaid Income Spend-Down is not an option. Therefore, this is where QITs come into play. As of 2022, the following states are income cap states, and Miller Trusts are an option to gain Medicaid eligibility: Alabama, Alaska, Arizona, Colorado, Delaware, Idaho, Indiana, Mississippi, Missouri , Nevada, New Mexico, Ohio, Oklahoma, Oregon, South Carolina, South Dakota, Wyoming.

    Appealing For Denial Of Coverage

Youll start receiving the appropriate Medicaid services if you meet all the requirements and are deemed financially eligible. If you dont meet all the eligibility requirements, you will be notified of your right to a fair hearing. States must provide individuals who think they should be eligible the opportunity to appeal the denial of coverage, either on the grounds that an error was made or that the state failed to act promptly.

The structure of the appeals process varies among states. The Medicaid agency may conduct the appeals process or otherwise delegate it. Another state agency may conduct the appeal following approval from the Centers for Medicare & Medicaid Services .

Q How Will I Receive My Medical Care

How to Apply for MO HealthNET (Medicaid in Missouri)

    A. In Delaware, Medicaid benefits are provided mainly through a managed care organization, or MCO, under contract with the state. Managed care is an organized way to ensure that people receive the quality medical care they need in the most cost-effective manner. Some Medicaid recipients in Delaware those who also receive Medicare and those in Long Term Care Medicaid programs such as the Nursing Facility program do not receive their medical care through a managed care organization.

DMMA provides a choice of managed care organizations so you can select the MCO plan that is best for you and your family. Delaware contracts with two managed care plans AmeriHealth Caritas and Highmark Health Options.

The Medicaid MCO provides almost all of the care for Medicaid members who join their plan. Prescription and non-emergency medical transportation services are covered directly by Medicaid, not through the MCO. Medicaid recipients need to show their Medicaid card to pharmacies and transportation providers when receiving these services.

    Establishing A Qualifying Income Trust

It is not necessary to use an attorney when creating a qualified income trust. However, for many families it might make sense to do so. There are inexpensive guides and documentation available online to help those wishing to undertake the process themselves.

However, because Medicaid eligibility laws are very complicated and specific to each state, one may find that these guides do not provide adequate detail for their situation or their geographic area. Since the consequence of an improperly established trust means a denial of Medicaid benefits, for most persons it is advisable to find a Medicaid planning professional to assist in the process.

    Community Cares Services Program

Generally people utilizing the CCSP Program through Georgia Health Services are elderly and unable to live alone. They do not need the level of care found in a nursing home but do require some assistance with everyday living. CCSP recipients have a monthly income between $400 and $2523 per month.

Persons served through the CCSP Program must be eligible for Medicaid and meet the following requirements:

    • Have cognitive and or physical impairments/limitations which make it difficult to perform normal daily living activities and live independently
    • Have health needs that can be met in the community with services offered by the program and within established individual cost guidelines.

What does this mean? If your loved one has Medicaid , meets the above criteria, and has an income less than $2523 dollars per month, they may qualify for the CCSP or SOURCE Program and obtain residency in a licensed personal care home.

How much does it cost? Residents using CCSP pay the personal care home $689 per month and a cost share determined by CCSP and DFCS.

Why do we have to pay a Cost Share? Cost Shares are used to offset the amount not paid by the State of Georgia for ALS Services. CCSP and DFCS determine the cost share.
