    The final rule released yesterday says that the Medicare payment increase reflects a 4.0 percent home health payment update percentage, which will add $725 million, and a 0.2 percent increase because of an update to the fixed-dollar loss ratio used in determining outlier payments, which will add $35 million.

    However, Medicare payments to home health agencies in 2023 will decrease by 3.5 percent after CMS determined that it has paid significantly more under the new Patient-Driven Groupings Model for home health agencies. The decrease will subtract $635 million from home health agency payments next year, with more cuts planned for subsequent years.

    CMS implemented the home health PDGM and a 30-day unit of payment in 2020. The PDGM is meant to better aligns payments with patient care needs, especially for clinically complex beneficiaries that require more skilled nursing care rather than therapy, according to CMS.

    To implement the model though, CMS had to make assumptions about behavioral changes because of the new unit of Medicare payment. The agency finalized three behavioral assumptions around clinical group coding, comorbidity coding, and the Low Utilization Payment Adjustment threshold.

    The cut next year will reflect what CMS would have paid under the old payment system and the remaining permanent adjustment will be proposed in future lawmaking.

    The average Medicaid reimbursement rate for home health and personal providers increased modestly in 2015, according to a new report from Kaiser Family Foundation distributed by the National Association of Home Care & Hospice.

    According to the report, home health agencies received an average of $93.93 per home health visit in 2015, an increase of $1.24 from 2014. In states that paid registered nurses or home health aides directly the average rate per visit was $87.26. In states that mandated reimbursement rates, the average rate per visit was $52.19. For the personal care services state plan option, the average rate paid to provider agencies was $18.82 per hour in 2015, a slight increase from $18.73 per hour in 2014. In states where personal care services providers were paid directly by the state or where reimbursement rates were determined by the state, the average reimbursement rate was $13.43 per hour in 2015.

    Medicaid Reimbursement Rates Vary By State

    One major distinction between Medicaid vs. Medicare reimbursement rates is that Medicaid reimbursement rates are state-run, while Medicare is nationally run. Also, State laws ensure that each Medicaid program is unique.

    However, most Medicaid reimbursement models pay providers through either managed care or fee-for-service or some combination of the two.

    Cms Backs Off Severe Cuts Finalizes 07% Increase To 2023 Provider Payments

    The U.S. Centers for Medicare & Medicaid Services released its FY 2023 home health final payment rule late Monday.

    It comes with an estimated increase to 2023 home health payments of 0.7%, or $125 million, compared to 2022 aggregate payments. Although it sets up certain financial challenges moving forward, the final rule is better news for providers than what was previously expected.

    The agency had initially floated a 4.2% aggregate decrease in 2023, which would have marked one of the steepest home health Medicare cuts in years. Nonetheless, CMS is still phasing in other cuts and permanent adjustments related to the rebalancing of the Patient-Driven Groupings Model .

    At least for now, immediate PDGM clawbacks appear to be delayed.

    CMS is phasing-in the permanent adjustment by finalizing a -3.925% permanent adjustment for CY 2023, the agency wrote in its fact sheet. The -3.925% permanent adjustment is half of the full permanent adjustment of -7.85% . This rule also discusses the comments received on the best approach to implement the statutorily required temporary payment adjustment for CYs 2020 and 2021, and those comments will be considered for future rulemaking.

    Strong pushback from providers across the country, as well as numerous home health advocacy organizations, seems to have paid off.

    Still, NAHC is not satisfied with the final rule, or CMS methodology.
