If your claim does get picked for targeted medical review, a third-party agency hired by CMS will analyze the claim in question. The claims inspector will verify that you provided appropriate services for the type and severity of your patients diagnosis at an appropriate duration and frequency. If documenting defensibly, your patients chart should be able to certify the patients level of complexity, severity, and that treatment beyond the threshold was necessary for the patients care.

If your practice has a high rate of claims errors on Medicare procedures, CMS may refer you to its Targeted Probe and Educate program. TPE gives you a chance to address issues with billing, but CMS may audit your practice if you dont successfully complete the program.

Medicaid coverage for physical therapy is covered for the categorically needy, there’s no copay, and patients get 144 units of service per year without PA.

What Parts Of Medicare Cover Physical Therapy

Part A covers physical therapy as an inpatient service in a hospital or skilled nursing facility if its a Medicare-covered stay, or as part of your home health care benefit. You are responsible for the Part A deductible for your hospital inpatient stay and a daily coinsurance if you are in a SNF longer than 20 days. There is no copay for physical therapy as part of your home health care benefit under Part A.

For instance, suppose you are in the hospital after surgery or after being treated for an acute illness like pneumonia. As you recover, physical therapy may be part of your treatment plan to ensure that you continue improving and functioning well once you are back home. Your physical therapist will provide hands-on care, education, and specific exercises you can do at home.

Sometimes, you will go directly home from the hospital and may continue to receive physical therapy services as part of your home health care benefit. Or, you may go to a Skilled Nursing Facility from the hospital to continue your rehabilitation and recovery before you go home.

Medicare Part C And Physical Therapy

When Will Medicaid Pay for a Nursing Home or Assisted Living?

Medicare Advantage is legally required to give you the exact same benefits as Original Medicare. The good news is that most Medicare Advantage plans offer even more than that. Your plan might charge lower coinsurance costs and/or copays. You might even be able to get physical therapy for free .

For better or worse, though, your plan can implement appointment caps and spending limits if they wish. And your Medicare Advantage medical network might not include the doctors or facilities that can give you the best care.

But medical networks and physical therapy limits can help you spend less money on your care. So there is a bit of give-and-take you have to accept with Medicare Advantage and physical therapy.

Who Pays For Medicare Part B Coverage

The people receiving care will first have to elect Medicare Part B coverage, which requires them to pay a premium. For 2021, this amount is $148.50 for those making $88,000 or less. There is also a deductible, which is $203 for 2021, that must be paid, either by the patient/resident or a co-insurer. The deductible can be paid to any provider of Medicare Part B services. In other words, prior to entering a facility, a senior may have already met his or her deductible elsewhere and will, thus, not have to pay it at the facility. Once that deductible is met, one will be covered by Medicare.

Medicare covers 80% of the fee schedules of Part B that will be detailed below. Certain states will pay the remaining twenty percent, or, the coinsurance amount. In most states, it is possible to get Medicaid to pay the Medicare coinsurance, but this process is not automatic in all states. Co-insurance is provided in accordance with state regulations. States that do not pay the coinsurance will most likely ask a long term care facility to write off the remaining amount.

Jimmo V Sebelius And The Maintenance Coverage Standard

Lastly, providers should also be aware of the Jimmo settlement agreement made by CMS in 2013s Jimmo v. Sebelius decision. The case was centered on providing patients with routine maintenance of their condition, showcasing that medically necessary care does not necessarily mean a patient will show visible progress. Under this standard, your services are medically necessary as long as they slow the patients deterioration or help maintain function.
