If you are a Provider and would like to participate in the Virginia Premier network, simply fill out the Recruitment Request Form below. If you would prefer to mail or fax your application to us, you can also download the Recruitment Request Form here.
Virginia Premier = Medicaid plans administered through the Department of Medical Assistance Services agreement.
Virginia Premier Advantage Elite = A Dual Eligible Special Needs Plan is a Medicare Advantage coordinated care plan specifically designed to provide targeted care to individuals for both Medicare and Medicaid.
