When Should I Call To Schedule My Ride
You can schedule your ride up to 30 days before your scheduled appointment. You must contact the transportation service at least 48-hours before your appointment. To schedule by phone, call 1-855-369-3723, Monday Friday from 7 a.m. to 7 p.m., excluding holidays. Or schedule your ride online.
Transportation requests for medical appointments not made at least 48 hours in advance may be denied. This 48-hour notice does not apply to rides to Urgent Care. It also does not apply to same-day appointments. A same-day appointment is when one provider refers you to another provider for an appointment on the same day.
How Do I Replace My Medicaid Id Card
Call Medicaid Customer Service at 1-888-342-6207 to request a new card or visit the online Medicaid Self-Service Portal. You can create an online account at any time. Until you receive your new card you should call your health care provider before your appointment to make sure they can verify your coverage.
If you need a new Healthy Louisiana plan ID card, call the member services department for a replacement card.
What Happens If My Trip Does Not Qualify
When you schedule your trip, the transportation service will first check to make sure you are enrolled with Louisiana Healthcare Connections. If you are not currently enrolled, you wont be able to schedule a trip.
If you want to schedule a trip for a regular medical appointment, you must call at least 48 hours before the appointment. Transportation requests for medical appointments not made at least 48 hours in advance may be denied.
When you schedule your trip, the transportation service will also ask the reason for your trip. Transportation is only provided to approved medical facilities and to COVID-19 vaccination locations. If you ask for a trip to a location that is not approved, the transportation service will deny your trip. They will explain why the trip is not covered. What If I Am Not A U S Citizen
To be eligible for Medicaid in Louisiana, you must be resident of Louisiana and either U.S. citizen or a non-citizen who is lawfully present in the U.S. Non-citizens can qualify for coverage for emergencies and labor and delivery services if income requirements are met.Medicaid and LaCHIP coverage may be available to children up to age 19 who are lawfully residing and are otherwise eligible.

How Does Medicaid Work
The Medicaid program does not provide health care services for everyone. Your application will be reviewed for all Medicaid programs. If you are found eligible for Medicaid, you will get:
- A letter in the mail from Medicaid telling you about your coverage
- A Medicaid ID card
- A Healthy Louisiana plan ID card, if you enroll in a plan.
Each time you go to your health care provider or pharmacy, take your Medicaid ID and Healthy LA plan ID cards with you. Show them at all medical visits before you receive any service. The health care provider or pharmacy will tell you if they accept your Medicaid benefits. You should also ask your health care provider or pharmacy if the service or prescription is covered by Medicaid before receiving the service or filling a prescription.
Medicaid does not pay money to you instead, it sends payments directly to your health care provider or pharmacy.

Health and fire-safety inspections with detailed and summary information about deficiencies found during the three most recent comprehensive inspections and the last three years of complaint investigations are found on the Medicare.gov Nursing Home Compare website.
Nursing Home Compare allows consumers to search for a nursing home based on location and compare the quality of care they provide and their staffing. Nursing Home Compare has detailed information about every Medicare and Medicaid-certified nursing home in the country.
For information about Louisiana nursing homes or any other nursing home inquiry, contact the LDH Health Standards Section, Nursing Home Program Desk at 225-342-0114 or visit the Louisiana Nursing Home Association website. Nursing home providers are required to post the results of their most recent inspection in the facility.
How Can I Identify My Driver
All drivers are trained to be courteous and wear an ID badge. Sign up for the text program to get an alert letting you know your drivers name and what kind of vehicle to expect. All drivers have passed strict quality and ADA sensitivity training to ensure they are safe and experienced drivers.
Passengers are expected to follow the instructions of the driver. They are required to wear seatbelts. The use of oensive language is not allowed. No smoking, eating, or drinking is allowed in vehicles.

Can I Get Transportation To Any Provider
Louisiana Healthcare Connections will provide transportation to providers up to a certain distance from your home. These limits are set by the Louisiana Department of Healths Louisiana Medicaid medical service area standards. If you want to visit a provider outside of these limits, you may have to provide your own transportation.
Primary Care Providers
- If you live in an urban parish, the limit is 10 miles .
- If you live in a rural parish, the limit is 30 miles .
- The limit is 60 miles without authorization.
Transportation to providers over these limits requires approval from Louisiana Healthcare Connections. Call 1-855-369-3723 and tell the transportation service why you need to see this provider. They will contact us to ask for authorization for your trip.

In order to participate fully in your therapies we encourage you to dress in comfortable, casual clothing. In addition, you should bring:
- Any items that were used on a regular basis prior to your illness, such as eyeglasses, hearing aids, dentures, and the appropriate containers
- Sturdy shoes or sneakers and loose socks, if appropriate
- Loose-fitting pants or sweatpants, shirts that are comfortable and easy to put on, undergarments, bathrobe, and pajamas

If Someone Else Drives Me To My Appointment Can They Get Reimbursed For Gas
Your friends or family members can get money for gas when they give you a ride to or from your doctors appointment. This transportation benefit is called Gas Reimbursement.
Your driver must live at a separate address. Gas reimbursement will not be paid to drivers who live at the same address as the member. Also, members will not be reimbursed for driving themselves to medical appointments.
Your driver must also enroll in the gas reimbursement program. Drivers need to submit their Social Security Number and copies of their drivers license, vehicle insurance and vehicle registration upon enrollment. The physical address on their drivers license must match the physical address on the drivers enrollment form. Once drivers are enrolled in the program, they can get payments sent directly to their bank account or by paper check. If a driver wants a paper check, they can choose to have it sent to their physical address or a P.O. Box address.
