In May 2014, Alabama submitted a Section 1115 demonstration waiver proposal to CMS, called Alabama Medicaid Transformation. The proposal called for Medicaid funds to be distributed on a per-patient basis to regional care organizations , most of which would be affiliated with local hospitals. The concept was that the RCOs could use preventive care and early interventions to keep patients out of the hospital.
