Community Health Choice Providers: The Community Health Choice provider enrollment steps are listed below:

  • Visit the Community Health Choices website.
  • A Community Health Choice portal is open. In this portal, enter your tax number on the pages of the secure access application after clicking on Register Now. After that, click Submit to successfully register on the Community Health Provider Portal.

Guide To Healthcare Services

Your Primary Care Provider is an important part of your healthcare team.

Your doctor will:

  • Make sure your child gets the care they need, like regular checkups
  • Treat your child when they are sick
  • Track everything about your childs past and present healthcare. Make sure they have all of your childs medical records. You need to sign a form giving permission.
  • Make referrals to other doctors, if needed

You can pick any doctor in the Community network as your Primary Care Provider. You should pick a doctor with an office location and office hours that are convenient for you. If you like the Primary Care Provider that you see now, you can continue to see them if they are listed in Communitys Network directory.

To find a doctor online, click on the Find a Doctor box below. You can also call Member Services at 1.888.760.2600, and we will find you at least three doctors in your area to choose from.

Once you have picked a doctor call Member Services. We will assign that doctor to you. We will also mail you a Member ID card with your new doctor information.

Automatic Extension Renewal

Effective February 1, 2022, Autism Services, to include Applied Behavior Analysis evaluation and treatment, became a benefit of the Texas Health Steps-Comprehensive Care Program for Texas Medicaid recipients who are 20 years of age and younger and who meet the criteria outlined in the Autism Services benefit description. This benefit is not available for CHIP recipients.

For your health and ours, we are not offering walk-in assistance at this time. Please call the number on your ID card to schedule an appointment or discuss other options for assistance. We will confirm your appointment and give you a phone number to text when you arrive. Save this phone number.

If you have any of these symptoms, please reschedule.

  • Shortness of breath or difficulty breathing
  • New loss of taste or smell

When you come for your appointment:

  • Come by yourself, if possible. Limit 2 people.
  • Text the phone number you received when you scheduled your appointment to let our Representative know you are here.
  • Wait outside until our Representative texts you back before entering the building.
  • Wear your face covering over your mouth and nose while you are in the building. If you remove it, we will not be able to assist you.
  • We will take your temperature and ask you a few COVID-19 questions when you come in. If your temperature is above 99.9 or your answers lead us to believe you may have COVID-19, we will not be able to assist you.

    To participate in the Community Health Choice , a member must:

    • Eligible for Medicare Part A
    • Registered with Medicare Part B
    • Live in our service area
    • Texas Medicaid Eligible Categories: QMB or QMB Plus.

    As a dual beneficiary, the beneficiarys benefits are paid first by Medicare and Medicaid. Medicaid coverage varies based on income, resources, and other factors. Benefits may include full Medicaid benefits and complete or partial payment of the members Medicare copayments .

    Community First Choice Services include:

    • Help with activities of daily living and health-related tasks through hands-on assistance, supervision or cueing.
    • Services to help the individual learn how to care for themselves.
    • Backup systems or ways to ensure continuity of services and supports.
    • Training on how to select, manage and dismiss attendants.

    Texas began the Community First Choice program on June 1, 2015. This means:

    • Individuals on a 1915 waiver interest list who meet eligibility and coverage requirements may be eligible to get Community First Choice services.
    • Individuals already getting services through a 1915 waiver will continue to get those services as they do today from their existing providers.

    Its important to choose a primary care physician from the Community Health Choice network to coordinate your care. Use our Provider Directory to see if your primary care physician is in the network or to find a new one.

    Make your selection by signing in to your Member account, go to the Message Center and email us the Provider First and Last Name/Clinic Name and phone number.     The community has partnered with Change Healthcare and ECHO Health, Inc. for Electronic payment and invoicing. We describe it below Payment options and all the promotional items your office needs:

    • Virtual Card Services: Virtual cards enable your office to accept payments such as credit card transactions and are typically received 7-10 days before paper checks as there is no delay in printing or sending. Your office will receive fax notifications containing a virtual card with a unique number for that payment transaction, your statement of payment , and a processing instructions page. Once you receive the virtual card number, enter the code into your agencys credit card terminal to process the payment.
    • EFT/ACH Setting up an electronic funds transfer is a fast and reliable method of receiving payments. If you want to sign up for EFT, you have two options:
    • To register to receive EFT through the Settlement Advocate for Community Health Choice only, visit https://view.echohealthinc.com/EFTERADirect/CommunityHealthChoice/index.html.
