How Medically Needy Is Related To Share Of Cost And Spend

Medically Needy describes a type of Medicaid program that looks at the individuals income and medical expenses as opposed to having a hard income limit which is called Categorically Needy. As mentioned above, one can spend excess income on medical expenses, bringing their income down to the MNA or MNIL, and therefore, qualify for Medicaid.

Medically Needy Medicaid may be called by many different names. However, to avoid confusion, they are all referring to the same medically needy pathway to Medicaid eligibility. As mentioned before, Medicaid Spend-Down Program and Share of Cost Program are two commonly used phrases for Medically Needy Medicaid. In Pennsylvania, it is called the Medically Needy Only Medical Assistance Program. And in Georgia, it is called the Adult Medically Needy Program. Other names by which it might be called include Aged, Blind and Disabled Medically Needy Program and Medicaid Deductible Program.

What Does The Literature Tell Us About The Effects Of Premiums And Cost

An earlier literature review found that premiums serve as a barrier to obtaining and maintaining Medicaid coverage for low-income individuals, with those who lose coverage facing increased barriers to accessing care. Research indicated that these effects are largest among those with the lowest incomes. Studies also found that even relatively small levels of cost-sharing are associated with reduced use of care and increased financial burden. Research suggested that state savings from premiums and cost-sharing in Medicaid and CHIP are limited, and that increases in premiums and cost-sharing can increase pressures on safety net providers.

A review of recent studies continues to support earlier research on the impacts of premiums and cost-sharing among low-income populations. We reviewed research from 24 papers published between April 2017 and May 2021 on the effects of premiums and cost-sharing on low-income populations ). Additional research on the impact of premiums from Section 1115 waiver reports are described in a later section of this brief. What Does The Data Show On The Impact Of Premiums Under Section 1115 Waivers

Medicare Cost Report Explained

In this section, we analyze data from five states that have implemented Section 1115 premium requirements on rates of premium nonpayment and consequences, reasons for nonpayment including confusion and affordability, and administrative burden. Our analysis is based on interim and final waiver evaluations as well as annual and quarterly state data reports posted on Medicaid.gov. Although we highlight key themes across states, comparison between states is limited by state-by-state variation in premium policies and available data metrics.

Available data from quarterly and annual state waiver reports suggests that high numbers of enrollees fail to pay premiums. Common reasons for nonpayment include confusion and affordability . For example, in Montana , for each month in 2019 an average of 57% of enrollees failed to pay that monthâs premium, while 74% had an overdue premium for a prior month these percentages were similar in 2017 and 2018.40 Lower-income enrollees in Montana were more likely to fail to pay: an average of 61% of those between 50% and 100% FPL failed to pay that monthâs premium versus 52% of those above 100% FPL .

Data indicates that many enrollees have faced an array of consequences for failure to pay premiums, including loss of coverage.

Oregon Medicaid Health Experiment And Controversy

In 2008, Oregon decided to hold a randomized lottery for the provision of Medicaid insurance in which 10,000 lower-income people eligible for Medicaid were chosen by a randomized system. The lottery enabled studies to accurately measure the impact of health insurance on an individualâs health and eliminate potential selection bias in the population enrolling in Medicaid.

A sequence of two high-profile studies by a team from the Massachusetts Institute of Technology and the Harvard School of Public Health found that âMedicaid coverage generated no significant improvements in measured physical health outcomes in the first 2 yearsâ, but did âincrease use of health care services, raise rates of diabetes detection and management, lower rates of depression, and reduce financial strain.â

The study found that in the first year:

  • Hospital use increased by 30% for those with insurance, with the length of hospital stays increasing by 30% and the number of procedures increasing by 45% for the population with insurance
  • Medicaid recipients proved more likely to seek preventive care. Women were 60% more likely to have mammograms and recipients overall were 20% more likely to have their cholesterol checked
  • In terms of self-reported health outcomes, having insurance was associated with an increased probability of reporting oneâs health as âgoodâ, âvery goodâ, or âexcellentââoverall, about 25% higher than the average

    Services Exempt From Cost Sharing

    Some medical services do not have a cost share. Services exempt from cost sharing include:

    How Is Share Of Cost Calculated

    MNILs are determined by the state. They can vary based on the number of people in your household.

    When calculating whether youve met your share of costs, Medicaid looks at any medical bills you have, paid or unpaid. Each bill can only be counted once towards your share of cost. This means that if you count an unpaid bill in one period, it cant be counted in the next period, even if thats when its paid.

    Paid bills can only be counted in the month that theyre paid, and they must be paid by you or someone who didnt have to pay it, such as your grown child. Any bill thats been paid for by Medicare or private insurance doesnt count toward your share of cost.

    Other expenses that can possibly be counted toward share of cost, depending on your state, include:

    • Bills for other members of your household if theyre for your spouse, children under 21 or a disabled person whos dependent on you
    • Cost of trips to the doctor, hospital or drugstore
    • Cost of medical supplies, such as needles or over-the-counter prescriptions
    • Cost of health insurance, such as Medicare and other health insurance premiums

    How Much Does Medicaid Cost And How Are Funds Spent

    Capitated payments to Medicaid managed care organizations and for other Medicaid managed care arrangements) account for 49 percent of Medicaid spending. In FY 2019, Medicaid spending totaled $604 billion. Managed care and health plans3 accounted for the largest share of Medicaid spending representing payments to comprehensive MCOs), 23 percent of Medicaid spending is for fee-for-service acute care, 21 percent for fee-for-service long-term care, 3 percent for DSH, and 3 percent for Medicaid spending for Medicare premiums and cost-sharing on behalf of dual eligible beneficiaries .

Although only approximately 5.5 percent of enrollees utilize long term services and supports, these enrollees account for nearly one-third of all Medicaid spending on benefits.
