Paying the Nursing Home While a Medicaid Application is Pending

Home health care can include skilled nursing or therapy services, home health aide services like medication management or bathing assistance, and personal care aide services like meal preparation or cleaning.

New Yorksâ managed care programs Program for All-Inclusive Care for the Elderly and Managed Long-Term Care both offer home health services as part of their benefit package, so if you are a participant in those programs, you can receive assistance from skilled nurses, home health aides, or personal care attendants in your home, as long as your doctor or care coordinator has authorized those services for you according to the requirements in your particular managed care plan.

In addition, New York offers personal care assistance to Medicaid beneficiaries through its Personal Care Services Program . PCSP covers assistance with in-home activities like housekeeping, bathing, using the toilet, and preparing meals. Your doctor must send a physicianâs order to your local social services district, and then a nurse assessor will visit your home and interview you about your activities of daily living and your need for assistance. The assessor will decide what kind and how much assistance you need. Generally, your local social services district selects the agency that will provide services to you.

To apply for Medicaid or any of these programs, contact your local department of social services in New York.

How Your Assets Impact Eligibility

Besides income, your assets will be counted toward meeting eligibility requirements. Countable assets include checking and savings account balances, CDs, stocks, and bonds.

In most states, you can retain up to $2,000 as an individual and $3,000 for a married couple outside of your countable assets. However, these amounts may vary depending on the state in which you live.

Your home, your car, personal belongings, or your savings for funeral expenses remain outside of countable assets. If you can prove other assets are not accessible , they too are exempt. A house must be a principal residence and does not count as long as the nursing home resident or their spouse lives there or intends to return there.

Upon becoming eligible for Medicaid, all of the applicant’s income must be used to pay for the nursing home where the applicant resides. However, you may be allowed to keep a monthly “allowance” and a deduction for medical needs, such as private health insurance. The amount of the allowance varies depending on your living arrangements, type of nursing facility, and state rules. If you are married, an allowance may be made for the spouse still living in the home.

Aunt Is Medicaid Pending But May No Longer Need Nursing Home Level Of Care What Happens Now

My 76-year-old aunt suffered a stroke in September and was sent to a nursing home for rehab. She has no assets or savings except a car, no home, no husband, no children, and her income is just slightly above the Medicaid limit. She lost her apartment in a flood just before the stroke and all her belongings are in storage.

At the time she was sent to the SNF, she needed help with ADLs and was newly diagnosed with diabetes requiring twice daily injections that she was unable to do herself. The situation at this nursing home has been highly chaotic. They are not only being investigated by the state for numerous violations, but COVID related issues have made it very difficult for them to retain enough staff and it’s almost impossible to talk to a social worker about her case. Her family never got a chance to meet with admissions when she was brought there .

I am confused as to why at first they wanted us to put her in assisted living but then they backed off and now are uncommunicative as she is in Medicaid pending status. I feel like they want to not re-assess her because they want to collect the Medicaid .

I just haven’t been able to find any reliable help or advice from the medical community because COVID means no one returns your calls… I want to change her situation though my family cannot afford assisted living for her, we can afford to take her in ourselves .

Determining Eligibility For Medicaid Long Term Care

Nursing Homes That Accept Medicaid Pending

Medicaid Eligibility Requirements FinderStart here

Regardless of the type of Medicaid program or the state in which a beneficiary receives it, there are two consistent eligibility criteria for Medicaid long term care. The individual must have limited financial resources and a documented need for care.

From a financial perspective, typically there are limits on monthly income, total countable assets and home ownership. These limits vary based on age, marital status, number of applicants in the family, state of residence and the specific type of Medicaid program. In addition, these limits change annually and, in some cases, twice annually.

Medical or functional eligibility criteria also vary by state and by type of Medicaid program. In all cases, the individual must be determined by Medicaid to require the type of care they are seeking.

Medicaid Benefits For Assisted Living

Currently, 46 states and Washington, D.C. offer some Medicaid coverage for assisted living expenses. The states that do not are Alabama, Kentucky, Louisiana, and Pennsylvania.

Because each state has authority over its own programs, the terminology used by state Medicaid administrators may confuse older adults and their families. If youre looking for coverage options in your states Medicaid benefits information, try searching the documents for phrases other states have designated for assisted living, such as residential care, supported living, alternative care facilities, adult foster care, and nonprofit personal care homes.

Assisted living residents with incomes above the maximum amount allowed for Medicaid recipients may still be able to receive Medicaids financial help through a variety of waivers available in each state. In many states, the allowance for increased income is as high as three times the typical maximum.

The 1915c Medicaid Home and Community Based Services and 1915b Managed Care waivers are among the most well known and frequently used, but their availability varies greatly from state to state.

There are assisted living costs specifically room and board that Medicaid simply does not cover, whether you receive Medicaid through financial eligibility or a waiver. If youre lucky, you may live in a state that places a legal limit on the amount assisted living communities can charge for services government programs dont cover.

Do All Facilities Accept Medicare And/or Medicaid Reimbursement For Care

Many skilled nursing and intermediate care facilities accept Medicare and/or Medicaid reimbursement, but residential care facilities do not. However, residential care facility residents may be eligible for state assistance through the Supplemental Nursing Care grant. In addition residential care facilities may participate in the Medicaid Personal Care Program. The Personal Care Program offers Medicaid-eligible residents assistance with activities of daily living. This program provides residential care residents an alternative to nursing facility care.

In What Locations Will Medicaid Pay For Long Term Care

THE BIG MEDICAID SECRET NURSING HOMES WON’T TELL YOU

Nursing Homes

Medicaid will pay for all medical care, non-medical care and room and board for a beneficiary that resides in a Medicaid nursing home. Nursing homes should not be confused with assisted living residences or memory care as those provide a lower level of care. Most, but not all, nursing homes accept Medicaid. In almost all states, Medicaid will pay for a shared room, not a private room unless that is a medical necessity.

Medicaid nursing home care is an entitlement. If someone meets the financial and medical eligibility criteria, the state must pay for their nursing home care.

Assisted Living

In most states, Medicaid will pay for some portion of a beneficiarys assisted living costs. However, in no states will Medicaid pay for the room and board portion of assisted living. That said, a state may have other non-Medicaid programs that provide assistance for rent.

Most assisted living residences do not accept Medicaid or have a limited number of Medicaid rooms. However, many assisted living residences allow outside caregivers to come in and provide care. These outside caregivers can be paid by Medicaid. Therefore, Medicaid beneficiaries who live in a non-Medicaid assisted living residence can still receive care paid for by Medicaid.

Paying For Nursing Home Care Before You Are Medicaid Eligible

If you are suffering from a serious illness or disability, you may eventually need to move into a facility that provides constant care, but that can be a problem without advance planning. While Medicaid covers the cost of a nursing home for those who qualify, unexpected problems during the lengthy application period may put your family in a difficult financial situation.

Who Regulates Nursing Homes

Nursing homes in Illinois are licensed, regulated, inspected and/or certified by a number of public and private agencies at the state and federal levels, including the Illinois Department of Public Health and the U.S. Department of Health and Human Services Health Care Financing Administration . These agencies have separate — yet sometimes overlapping — jurisdictions.

How Much Will Medicaid Pay For Nursing Home Care

For qualifying individuals, Medicaid picks up most of the tab for nursing home care, paying an average of 45 percent to 65 percent. However, coverage is largely dependent on a variety of factors such as financial status, the state you reside in, and the level of care needed.

Medicaid coverage amounts may vary from state to state, depending on the demand for nursing home care coverage. For example, the average cost of nursing home care in the state of California is on the higher end at $304 a day, compared to Texas at $165. This greatly impacts the coverage and benefits you can receive from Medicaid to offset the costs of nursing home care.

A: Have A Professional Informal Eligibility Assessment

Medicaid Assistance in Nursing Homes

Once you have made the decision that the applicant will be financially eligible, it is recommended you confirm this assessment with a professional. Submitting your application correctly the first time is a lot of work, but its still much easier than having to appeal a denial. Any mistakes make a delay or denial of benefits more likely, even if the applicant is eligible.

One way to get help is a benefits counselor at your local Area Agency on Aging. AAA offices are meant to help with exactly this sort of issue for older adults, and they will be experts on your specific state requirements. Theyre also free. The downside of getting help from AAA, however, is that theyre busy and may not be able to assist as soon as youd like. To find your local AAA office, .

Another option to get help with your Medicaid application is a Certified Medicaid Planner. Unlike the AAA benefits counselor, CMPs are paid services. CMPs will, however, offer an initial consultation for free. https://cmpboard.org/ For much more on Medicaid planning, see below.

Medicare Vs Medicaid Roles In Nursing Home Care

Medicare does cover nursing home careup to a point. If you are sent to a skilled nursing facility for care after a three-day inpatient hospital stay, Medicare will pay the full cost for the first 20 days. For the next 100 days, Medicare covers most of the charges, but patients must pay $185.50 per day unless they have a supplemental insurance policy. For day 101 and beyond, the patient pays all costs.

These rules apply to traditional Medicare. People on Medicare Advantage plans likely have different benefits

Some nursing homes wont accept Medicaid patients outright, but the law forbids them from throwing you out if you become dependent on Medicaid when you are in their care.

How And When Medicaid Can Pay For A Nursing Home

How Single Person Protects Assets From Nursing Home – Medicaid Planning: Part 6 of 6 – Applying

Medicaid was created in 1965 as a social healthcare program to help people with low incomes receive medical attention. Many seniors rely on Medicaid to pay for long-term nursing home care.

Most people pay out of their own pockets for long-term care until they become eligible for Medicaid. Though Medicare is an entitlement program, Medicaid is a form of welfareor at least thats how it began. So to be eligible, you must become impoverished under the programs guidelines, says Laura M. Krohn, a Rhode Island-based elder law attorney.

Let’s look at how the economics work, and how Medicaid can be used to pay for a nursing home.

What Is 100% Private Pay

Private assisted living includes homes or facilities that are privately owned rather than affiliated with a hospital or another medical organization. They are geared towards seniors who may no longer be able to live at home, but do not need the level of medical attention offered at, for example, a nursing home. Its important to know that Medicare typically does not pay for assisted living. Medicaid coverage is also highly unlikely, although it depends on individual state regulations.

Assessing Medical Eligibility For Nursing Home Medicaid

In every state, an individual receiving Nursing Home Medicaid must require a Nursing Facility Level of Care . Because nursing homes are expensive, with average costs in the U.S. at more than $250 per day, Medicaid does not want to pay for someone to receive nursing-level care outside the home if it isnt absolutely necessary. It may be much more affordable to make home modifications like installing grab bars or security systems for remote monitoring, and Medicaid will pay for these through programs like Home and Community Based Services waivers, if your loved one qualifies, rather than an expensive move into nursing care.

Assessments are how it is determined someone needs NFLOC and is therefore functionally eligible for Nursing Home Medicaid.

Pre-Admission Assessments for Nursing Home CareEvery state has its own standard screening process to decide whether Medicaid applicants need a nursing home. A general term for the required screening is pre-admission level of care screening. There may be a simple questionnaire you fill out at home, or a more involved evaluation with your physician, as some states have more levels of assessment requirements than others. A Medicaid applicant might need help with a specific number of ADLs, or a doctors diagnosis of a specific medical condition might be required. Because there is so much variance, someone who is eligible for Medicaid in one state might not be eligible in another.
