How Does A Family Apply

  • Applications can be completed online at colorado.gov/PEAK. The website features a streamlined application process for faster eligibility determination and the opportunity to print insurance cards online before receiving them in the mail.
  • To apply for assistance in person, pick up an application from either our Glenwood Springs office or our Rifle office, call to make other arrangements to obtain an application. Walk-in interviews are available Monday through Friday from 9 am 4 pm in our offices in Glenwood Springs, 108 8th Street, Suite 300, and Rifle, 195 West 14th Street.
  • You can print the Application for Public Assistance from Colorado Department of Health Care Policy and Financing.
  • Applications are available in English and Spanish

Special services

Children under the age of 21 are eligible for special benefits through the EPSDT program. Children are automatically referred to EPSDT after enrollment in Medicaid.

Health First Colorado Program Who Might Be Eligible

  • Individuals and families who meet income limits

  • People ages 16-64 who are working and have a disability
  • Health First Colorado Buy-in Program for Working Adults with Disabilities provides adults with disabilities, who earn too much income or have too many resources the opportunity to purchase Health First Colorado benefits.   • People who are eligible for Supplemental Security Income .
  • If you receive SSI, then you will automatically receive Health First Colorado. If you receive SSI and have not been enrolled in Health First Colorado, please contact your local county department of human/social services.
  • If you have not yet applied for SSI, you should apply at a Social Security Administration office.
  • People who receive Old Age Pension grants who do not qualify for other Health First Colorado programs. This program provides limited medical care only.
  • If you need nursing home or home and community based services , please see below for information about Long-term Care Medicaid.

    People Age 65 and Over: People age 65 or older who are eligible for Supplemental Security Income and/or Old Age Pension state supplemental payments are eligible for Health First Colorado.

    Update On Cash Assistance Program: Over $17645m Sent To 504421 Medicaid Peachcare For Kids Snap And/or Tanf Recipients Over 582138 Successful Transactions

    Atlanta, GA Following Governor Brian Kemps announcement that more than $1 billion in ARPA funds would be directed to the Department of Human Services to provide cash assistance of up to $350 for qualifying Georgians, the program launched on Sept. 20th. Within the first three days, 272,873 Georgians on Medicaid, PeachCare for Kids®, SNAP, and/or TANF have claimed $95,505,550 and spent $30,090,461.42 through 582,138 successful transactions. Records indicate the top purchase categories are retail stores, utility bills, and grocery stores. These funds can be used for online purchases or added to digital wallets for in-store use where accepted. The cash assistance cards cannot be used to buy alcohol, gamble, game, buy a money order, or transfer the balance to Apple Cash, Cash App, or Zelle, among other reasons listed in the cardholder agreement. Also, in-store Walmart purchases using the funds on Apple Pay will be declined since Walmart does not accept Apple Pay. The top two categories for declined transactions thus far are restricted purchases and attempts to convert the card to cash. As cash assistance payments continue to be issued in the coming weeks to eligible Georgians statewide, we ask people to be aware of the following:

    Emergency Rental Assistance Program

    El Paso County residents can apply for rental assistance through the state-run Emergency Rental Assistance Program . El Paso County partnered with the Colorado Division of Housing to administer the program that provides assistance with rent and rental arrears to eligible tenants and landlords who have experienced financial hardship due to the COVID-19 pandemic.

    El Paso County tenants and landlords can learn more about the program and apply by visiting the DOH website at . Applicants will have to register through the online portal to apply. Application and required documentation must be submitted online. DOH staff will review all applications and make direct payments on applicants behalf. A news release about the program with a brief overview of eligibility is posted online.

    Peakhealth Mobile App For Health First Colorado & Chp+ Clients Now Available

    The Department of Health Care Policy and Financing has launched the new PEAKHealth mobile app. PEAKHealth gives Health First Colorado and Child Health Plan Plus members a simple way to keep their information up to date and access important health information right from their phone. PEAKHealth is for current Health First Colorado and CHP+ clients who have a Colorado.gov/PEAK account. PEAKHealth is not for people who want to apply for benefits.

    PEAKHealth will help Health First Colorado and CHP+ clients to search for a provider, view their medical card, update their income and contact information, view benefit information, make a payment and access health and wellness resources.
