Future Of Medicaid At Walgreens

Drugstore giant Walgreens announced this week in the lagging economic climate it expects increased pressure from states regarding Medicaid reimbursements for drug prescriptions.

As states wrestle with budget difficulties and the need to reduce overall spending, we do expect pressure to continue on Medicaid reimbursement, Walgreens President and CEO Greg Wasson said Tuesday during the companys second quarter earnings conference call.

Medicaid beneficiaries typically pay between $1 and $3 per prescription. However there are exceptions, such as for children or pregnant women, who do not have to pay at all. Also, if the beneficiary cannot afford to pay the fee, the pharmacy must still dispense the drug. But if states cut reimbursement payments to the pharma giant, the chain might threaten to stop carrying the government-backed health insurance plan, according to Sellers Dorsey Senior Consultant Kip Piper.

State agencies set the reimbursement rates, which determine how much a pharmacy will receive from the state for each Medicaid prescription it fills. Piper said Medicaid historically has reimbursed better in terms of dispensing fees than private insurance companies, however, with states tightening their budgets across the country, this may not continue to be the case.

Walgreens spokesperson Tiffani Washington said this government budget-tightening issue is facing all pharmacies across the country that honor Medicaid prescription plans.

Walgreens To Quit Serving Medicaid In 44 Drug Stores In Washington State

DEERFIELD, Ill. With negotiations between retail pharmacy leaders and the state of Washington at an impasse over the states refusal to reconsider a deep cut to its Medicaid prescription reimbursements, Walgreens has thrown down the gauntlet. The chain announced Monday it would withdraw 44 of its pharmacies from the state’s Medicaid program as of May 1.

Walgreens operates 111 pharmacies throughout Washington, but the 44 pharmacies in question represent more than 60% of its total Medicaid business in the state, the company said.

Under new Medicaid reimbursement rules set to take effect April 1, Washington will significantly reduce the price it will pay for brand name medications. Those cuts, Walgreens announced Monday, will have the effect of severely impacting the economic viability of doing business in Washington.

Concern over the Medicaid payments to pharmacies erupted Friday, March 27, when the National Association of Chain Drug Stores and the Washington State Pharmacy Association warned in a joint statement that the payment cuts could force 72 pharmacies to choose between losing money to provide medications and services to their patients on Medicaid, and closing their doors. Adding insult to injury, the cutbacks come as the state prepares for a $1.8 billion infusion of economic stimulus money from the federal government for its Medicaid program.

Kermit Crawford, Walgreens senior VP of pharmacy, explained the companys move.

As of February 1, 2019, Superior HealthPlan members will no longer be able to use Walgreens Pharmacy for their pharmacy prescription services, including Over-the-Counter Medicaid-covered drugs. This change affects members in the STAR, CHIP, STAR+PLUS, STAR Kids and STAR Health programs.

This means that Walgreens Pharmacy will not be able to provide medications through Superior. Members will still have access to a large provider network and Superior will work with members to ensure they receive the care they need.

Please let patients know that changing medications to a drug store that is in the Superior network is easy. Members can do this in one of three ways:

  • If they have refills on the prescription, the member can take prescription bottles to a new in-network drug store to request transfer from store to store. The prescription must be in date and transferable. Their local pharmacy can help them understand if that will work for them.
  • Have their doctor call in a new prescription to the new drug store.
  • Bring a written prescription from their doctor to a new drug store.
  • Please note: Certain medications such as CII are not transferable and generally require a new prescription.

    The Walgreens Prescription Savings Club does come with certain restrictions.     • Certain states are not included: As of January 1, 2020, Prescription Savings Club memberships will no longer be sold or accepted in Connecticut, Massachusetts, and Mississippi.

    • Insurance might be cheaper: Compare your health plans price against the Walgreens price. Your drug might actually cost less with your insurance.
