Your Medicare coverage impacts which new primary doctor you can select. Here are the options for the various parts of Medicare and Medicaid:

During those three months, you should find a Medicare provider to prescribe your medications whoâs enrolled in Medicare or who has an opt-out request on file.

If you have moved to a different place, your first order of business is to contact your local Medicaid office in Texas and inform them of your address change within 10 days of moving. Log in to your YourTexasBenefits.com Account and update the address. If you dont have such an account, you should create it first and include your new address. You could also visit the website of your health plan provider and use their finder tool to search for compatible PCPs within their network.
When you visit the new PCP for the first time, ensure you are open about your medical history to make it easier for them to provide the care you need.

Medicare Advantage HMO members typically must choose primary care doctors from their plans network. You can do so by visiting your plans website and browsing through the doctors listed in your network. Your insurance company can also provide you with a hard copy through the mail if you prefer.
Take your time to decide and compare each doctors credentials and areas of specialty. Some insurers also provide patient reviews that are worth evaluating.
To change doctors:
- Choose a doctor from the network list.
- Ask the practice whether the doctor is accepting new patients and make an appointment if possible.
- Contact your insurance company through the phone number on your plan ID card to notify them of your new primary health care provider. In some cases, you may be able to make the change online.
- Confirm if your new doctor obtains your medical records or if you must coordinate this transfer.

You can change your primary care provider for several reasons.
As previously stated, if you dont trust your PCP and feel that they are not providing the right kind of medical care, you can part ways with them.
If you opt for a new Medicaid healthcare plan and your current PCP is not part of its network, it may make more sense to find another provider within the network for your own convenience. Otherwise, you will spend a lot of money paying out-of-pocket medical expenses your plan should be covering.
If you move to a different area, and your PCP doesnt have a local branch, you have no choice but to find a different primary care provider to serve your healthcare needs.

New Primary Care Provider Change Request Form For Members In Managed Care Prepaid Health Plans
In collaboration with primary care providers , the NC Medicaid Managed Care Prepaid Health Plans have created a new standardized PCP Change Request Form for members who wish to change their primary care assignment throughout the year. A link to each health plans form can be found here:
As a reminder, beneficiaries can change their PCP/Advanced Medical Home without cause twice each year.
Below are the different ways beneficiaries can change their PCP/AMH:
- Beneficiaries can select a PCP/AMH at application, recertification or through choice counseling with the Enrollment Broker when they change a health plan.
- Once assigned a health plan, beneficiaries should contact their health plan to change their PCP/AMH using this form or by calling each plans Member Services directly. Contact information can be found in the Member Handbook on the Health Plan Contacts and Resources Page.
- Beneficiaries can change their PCP or health plan at any time over the course of the year if they have care or quality concerns. This is known as a change with cause. Original Medicare And Medicaid
Find the doctors near where you live who accept new Medicare or Medicaid patients, including their office locations and specialties. You can visit the Federation of State Medical Boards to check the doctor’s credentials and if they have any actions against them.
To change doctors:
- Find a qualifying doctor who is accepting new Medicare or Medicaid patients.
- Contact the doctors practice and confirm they are accepting new Medicare or Medicaid patients.
- Make an initial appointment with your new doctor and transfer your care to them.
For seamless continuation of care, it may be recommended to see the same doctor at each appointment. However, Medicare beneficiaries arent required to select primary doctors. Because of this freedom of choice, theres no need to notify Medicare or Medicaid if you change doctors.
You also dont need to notify your former doctor of your decision to leave, although you may like to. However, its prudent to check if your new doctors office obtains your medical records or if that is your responsibility.
If they do not provide this service, you should see your previous doctor to request these records and have a health status check before moving to a new doctor.
