Private health insurance plans may pay for select elder care services, but coverage varies from plan to plan. Most forms of private insurance will not pay for non-medical home care services, and in-home skilled care is rarely covered at 100 percent. Consumer Direction is a way of getting some of the home care you need that offers more choice and control over WHO gives your home care and HOW your care is given than receiving CHOICES services and not consumer directing. You actually employ the people who provide some of your home care services – they work for you . This means you must be able to do the things that an employer would do – like hire, train, and schedule workers.

You can hire a family member, friend, neighbor, or other person you know to provide care in Consumer Direction, but there are some limitations. We want to support, but not replace, care already provided by family and friends. You’ll find out more if you sign up for CHOICES.

How Does Medicare Cover Home Health Care

Medicare Part A and/or Part B may cover home health aides if your doctor orders this care for you and determines that its medically necessary. Usually Medicare wont cover a full-time, long-term home health aide. The care must be part-time and provided from time to time not constant.

    If your physician orders reasonable and medically necessary treatment of an illness or injury, Medicare will help pay for home health services as long as you meet certain criteria. To be eligible for home health services, your condition must be expected to improve in a generally predictable period of time. You may also qualify if you need a skilled therapist to safely and effectively create a maintenance program and to perform maintenance therapy for your condition. A registered nurse or licensed practical nurse may administer your home care which may include giving IV drugs, changing wound dressings, tube feedings, teaching about diabetes care, or certain injections.

    Home Health Services are Medicaid State Plan services provided on a part-time and intermittent basis to Medicaid consumers of any age. Home health services include home health nursing, home health aide, and skilled therapies . The only provider of home health services is a Medicare Certified Home Health Agency .

    The medical necessity for home health services must be certified by the consumer’s qualifying treating physician. A face-to face encounter with the consumer and the physician, advanced practice nurse in collaboration with the physician, or a physician assistant under the supervision of the physician is required for certification of medical necessity.     If you enroll in one of the above waiver or PACE programs after you have been in a nursing home for more than 90 days, then you may be entitled to additional services aimed at transitioning you back to your home. You can get assistance making modifications to your house and with temporary rental payments while home modifications are being completed. These services are provided under Virginia’s Money Follows the Person program, a demonstration project administered by the Virginia Department of Medical Assistance Services. You can get these services if you are moving into your own home or apartment, your family’s home or apartment, or a small community-based group home containing no more than four unrelated people.

The Medicare home health care benefit is only for intermittent, part-time services for less than 21 days.     What Is Medicare Skilled Nursing Care

    According to the Centers for Medicare & Medicaid Services , skilled nursing care is care given by a nurse or other trained health care provider who can administer IVs and other injections, change out wound dressings, manage medication doses and more. Coverage is good for a certain number of skilled nursing care hours per day and per week.

    When skilled nursing care is needed, the benefits also extend to personal care, or custodial care. This includes assistance with activities of daily living like bathing, dressing, toileting, eating, mobility and more.

    Other home health care services covered by Medicare include physical, occupational and speech therapy, along with social services like counseling.

    Does Medicare Advantage Cover Home Health Aide Services

    Medicare Advantage plans are offered by private insurance companies. They must provide the same coverage as Original Medicare at a minimum. Some many plans offer additional benefits to their members, including expanded coverage for home health care.

    Some Medicare Advantage plans may include limited home health benefits. This coverage might include coverage for at-home meal and grocery delivery, transportation to and from doctor visits, and home health aide services for custodial care not typically covered under Original Medicare.

    Not all Medicare Advantage plans offer these benefits, and those that do may have limitations on how the benefits can be used. For example, the plan may choose to cover up to 50 hours per year of home health aide services, or 20 transportation trips per year. They may also require you to use a network home health agency or transportation provider. You may have a copayment or coinsurance amount for these services.
