Copays For Medical Assistance And General Assistance Members

What is Medicare Medicaid and how much does it cost?

Medicaid members 18 years of age and older and in the Medical Assistance or General Assistance categories will have to pay a copay for prescriptions and various medical services.

Members who are under the age of 18, pregnant, or in a nursing home do not have to pay the copays.

Residents of a long-term care facility or other medical institution, including intermediate care facilities, do not pay copays.

MA recipients, regardless of age, who qualify for benefits under Title IV-B Foster Care and Title IV-E Foster Care and Adoption Assistance do not pay copays.

PCP visits never have a copay.

Medical and General Assistance recipients cannot be denied a prescription if they cannot afford a copayment. If you cannot afford your prescription copayment, please let your pharmacist know. If you have any problems getting your medication from the pharmacist, please contact Member Relations at 1-800-553-0784 or 215-849-9600 .

Characteristics Of The Enrollees

Table 2 provides descriptive data on the enrollees in the Plus and Standard samples. Consistent with the propensity score matching on these measures, only very small and statistically insignificant differences are evident. The use and expenditure measures indicate consistently higher rates of use and expenditures among the Plus sample. This reflects the differences in these populations inherent in their OHP eligibility characteristics and underscores the fact that this is a nonequivalent comparison group. The sample selection criteria result in samples from both groups that are older and have a higher prevalence of chronic illness and behavioral health service use than the underlying population.

Insurance Factors That Affect Costs

What is it?

What it helps cover:

  • Medicare Part D covers prescriptions drugs.
  • Plan premiums, the drugs that are covered, deductibles, coinsurance and copays vary by plan, so you should compare plans each year based on your needs, the prescription drugs you take, etc.

What it costs:

  • Like Medicare Advantage , prescription drug plans are offered by private insurance companies contracted by the federal government.
  • Plans vary in cost, coverage, deductibles and copays.
  • Theres a late enrollment penalty if you dont enroll in an approved Medicare drug plan when youre first eligible, unless you have creditable prescription drug coverage through another Medicare health plan that offers Medicare prescription drug coverage.
  • Medicare calculates the penalty by multiplying 1% of the national base beneficiary premium times the number of full, uncovered months you didnt have Part D or creditable coverage.
  • This penalty is assessed when you enroll, and youll pay the higher amount for as long as you keep your Part D coverage
  • Your Part D premium could be higher depending on your income.

Medicaid Offers Additional Mental Health Support

Medicaid is the single largest payer for mental health services in the United States. While they didnt always include therapy as a benefit and limitations do still exist, Medicaid now covers this and a myriad of other substance use, mental health, and behavioral health services.

If youre enrolled in a Medicaid plan, some of your covered services may include:

    • Psychological testing and evaluation
    • Social work services
    • Case management for chronic mental illness

Other services like tobacco use cessation, mental health skill-building, and peer support should also be covered. If youre experiencing mental health concerns or battling a substance use disorder, its important to seek professional care and get the diagnosis and treatment you need.

If you or a loved one are struggling with substance use or addiction, contact the Substance Abuse and Mental Health Services Administration National Helpline at 1-800-662-4357 for information on support and treatment facilities in your area.

Doctor Visits And Medicare Supplement Insurance

It may be useful to know that Medicare Supplement insurance plans may help pay for Medicare Part A and Part B out-of-pocket costs. Medicare Supplement insurance plans generally pay at least part of your coinsurance amounts for Medicare-covered doctor visits. Most standardized plans typically pay the full Part B coinsurance amount.

For example, suppose you had a doctor visit, and the doctor ordered an MRI screening. Lets say the Medicare-approved costs were $100 for the doctor visit and $900 for the MRI. Assuming that youve paid your Part B deductible, and that Part B covered 80% of these services, youd still be left with some costs. In this scenario, youd typically pay $20 for the doctor visit and $180 for the x-rays.

If you had Medicare Supplement Plan M, those Part B out-of-pocket costs might be completely covered so you would pay nothing. Of course, Medicare Supplement plans come with a monthly premium. But if you have many doctor visit costs, you might want to learn more about Medicare Supplement plans.

Medicaid Healthy Michigan Plan Members Who Enroll In A Health Plan Will Pay Most Of Their Co

Now that you have Healthy Michigan Plan Medicaid and you have chosen your Medicaid health plan, how much will you pay and how will you pay it?

There are two costs associated with Healthy Michigan Plan Medicaid co-pays and contributions. Co-pays are small payments for services and prescriptions. Your co-pays will be calculated every 3 months based on how much you went to the doctor and what services you received. Your contribution is like a premium you would pay for health or other insurance. If you owe a contribution, it is charged every month and is based on your income. Co-pays and contributions are paid quarterly, either by writing a check and including payment coupons or by paying online through a MI Health Account.

What Factors Determine Medicaid Prices For Prescription Drugs

Medicaid payments for prescription drugs are determined by a complex set of policies, at both the federal and state levels, that draw on price benchmarks . States have some flexibility within federal guidelines to set the payment amounts. Price benchmarks represent prices paid by different parties at different stages in the drug manufacturing, acquisition, and dispensing processes. Because price benchmarks are related to one another, the prices paid throughout the drug distribution process have an effect on the final price that Medicaid pays.

State Medicaid programs reimburse pharmacies for prescription drugs based on the ingredient costs for the drug and a dispensing fee for filling the prescription.3 Medicaid agencies do not buy drugs directly from manufacturers. Rather, they reimburse retail pharmacies that fill prescriptions written for Medicaid enrollees. The amount the pharmacy receives is based on the ingredient cost of the drug and professional dispensing fees paid by Medicaid, plus any cost sharing paid by the beneficiary . For beneficiaries who receive their drug benefit through managed care organizations , MCOs reimburse the pharmacy, usually through the use of a PBM.

    Cost Sharing

The copayment amount for physician office visit is $1.00 per visit. Copayment does not apply to services provided for pregnant women, nursing facility residents, recipients less than 18 years of age, emergencies, and family planning.

The provider may not deny services to any eligible Medicaid recipient because of the recipients inability to pay the cost-sharing amount imposed.

    COINSURANCE/DEDUCTIBLE AND/OR COPAYMENT

Medicaid responsibility for payment of coinsurance/deductible and/or copayment amounts is:

    Coinsurance, Copayments, andDeductibles

Medicaid pays the appropriate coinsurance amounts, copayment amounts, and deductibles up to the beneficiarys financial obligation to pay or the Medicaid allowable amount , whichever is less. If the other insurance has negotiated a rate for a service that is lower than the Medicaid allowable amount, that amount must be accepted as payment in full and Medicaid cannot be billed.

Medicaid services not covered by another insurance

If the other insurance does not cover a service that is a Medicaid-covered service ,Medicaid reimburses the provider up to the Medicaid allowable amount if all the Medicaid coverage rules are followed.

MDHHS cannot be billed for copays, coinsurance, deductibles, or any fees for services provided to beneficiaries enrolled in a MHP, or who are receiving services under PIHP/CMHSP/CA capitation.

Im Just Going To Change Pharmacies

    • Remember, the price of your prescription is usually set by your insurance, not by the pharmacy.
    • There are cases where your insurance has signed a deal with a certain pharmacy company. Ask your insurance if that is the case.
    • If your not saving money, changing pharmacies can be dangerous. Sticking with one pharmacy allows that pharmacy team to get to know you and your drug therapy. They will be able to help you the best.
    • Instead of changing, work with your pharmacist to find more cost effective alternatives.

Doctor Visit Charges Are Not Set In Stone

It is important to remember that, even today, doctor visit fees are often negotiable, both before and even after an office visit. Discounts are widely available for self-payers, and like any other consumer product or service, individuals can shop around to locate the best price for a particular procedure especially if they know in advance what the average charge is for a particular service, or what other providers in the area are asking.

Also, some physicians have decided to opt out of the current insurance payment model and have returned to a less administratively complex, fee-for-service approach. Others have elected to do away entirely with fees, charging their patients one annual price for any and all services rendered during the year.

Patients who are having trouble paying their medical bills, including doctor visits, may benefit from talking to a professional debt relief company experienced in negotiating payments on their behalf. Either debt settlement or debt consolidation may be an appropriate option to help settle overdue or unmanageable accounts.

Patients who qualify for Medicaid, which is the federal/state insurance program for low-income individuals and families, can have their doctor visits covered. Eligibility requirements vary from state to state.

    Determining Your Medicaid Co

As mentioned before, instead of having a standard co-pay, the amount covered by Medicaid is based on your income. In order to determine your individual Medicaid co-pay, start by taking your total monthly income from all sources. Next, subtract your Personal Needs Allowance. This allowance varies by state but is typically around $50. Then, deduct any qualified medical expenses, such as health insurance premiums. And finally, if you have a community spouse at home who is eligible for an income shift based on the Monthly Maintenance Needs Allowance rules, deduct that amount as well. The resulting amount is then paid to the facility as your Medicaid co-pay, and Medicaid covers the rest.

The deductible is the amount you are required to pay in a given year or benefit period before Medicare begins paying its share.

What If I Think My Copay Is Wrong

    Co-Pay Covered Expenses

If you feel your copay is too high in light of the income information youâve provided to the Medicaid program, you may appeal the decision. However, it is most useful to contact the doctorâs office first to explain your situation see if charges can be modified.

If there is no resolution after this first step, Medicaid recipients who live in Pennsylvania can contact the County Assistance Office and explain their situation. If the County Assistance Office cannot resolve the issue, then they will refer it to the stateâs Office of Medical Assistance Programs in Harrisburg.

If the Office of Medical Assistance Programs reviews your complaint and finds you were correct, they will contact the provider and ask them to reimburse you for any payment already made.
