Drugs With Ind Status
INDs are medications in development that have not yet been approved for marketing by the FDA Because AIDS is a life-threatening disease, people with HIV infection and their advocates have exerted pressure to make drugs with IND status more widely available . They argue that given the high mortality rate for AIDS, and the increasing incidence of HIV infection, promising AIDS-related drugs should be made available to the medical community and to patients with HIV-related conditions as soon as possible .
The questionnaire also asked each Medicaid program: If drugs with IND status are provided to Medicaid recipients by drug companies at no charge, will Medicaid pay for the medical services necessary to administer the IND to the Medicaid patient? As Table 3 presents, the Medicaid programs were about evenly divided between States that covered the medical services necessary to administer drugs with IND status and States that did not. The South Dakota Medicaid program noted that generally these services would not be covered, but exceptions are made for people with AIDS and HIV infection. The Medicaid programs in the States of Washington, Minnesota, and Vermont responded that other medically necessary services must also be provided in order for Medicaid to cover the medical services necessary to administer INDs.

How To Request A Medicine Not On The Pdl
You can ask for a medicine that is not on our preferred drug list. This is called asking for an exception. To ask for an exception, please fill out an exception request form and submit it. Select your state from the list above and click the GO button to get the link to this form. When you are done, click Submit Request. Our Pharmacy department will look at your request and give you an answer within 72 hours

What Is A Prior Authorization And How Do I Get One
A Prior Authorization is a request from your provider for Health First Colorado to cover a drug not listed on the Preferred Drug List or otherwise listed on Appendix P of the Preferred Drug List. Your provider will submit the prior authorization request for you. After your prior authorization request is reviewed you and your provider will find out Health First Colorado’s decision. If the criteria are not met, the doctor can re-submit with updated information, or appeal the decision to Health First Colorado’s Pharmacy Benefits section for further review.
Each request is processed within 24 hours, and most phone requests are given the approval/denial decision immediately upon submission. If you have an emergency, your provider can get a 72-hour emergency supply of the drug you need.

Before Taking This Medicine
You should not use Valtrex if you are allergic to valacyclovir or acyclovir .
To make sure Valtrex is safe for you, tell your doctor if you have:

kidney disease
-
HIV/AIDS, or other conditions that can weaken the immune system or
-
a history of kidney transplant or bone marrow transplant.
While Valtrex use in pregnant women is limited, there is no evidence of major risk to the human fetus from valacyclovir. However, herpes virus can be passed from an infected mother to her baby during childbirth. If you have genital herpes, it is very important to prevent herpes lesions during your pregnancy, so that you do not have a genital lesion when your baby is born.
Valacyclovir can pass into breast milk and may harm a nursing baby. Tell your doctor if you are breast-feeding a baby.
Do not give this Valtrex to a child without medical advice.

What Other Drugs Will Affect Valtrex
Valtrex can harm your kidneys. This risk may increase when you also use certain other medicines, including: antivirals, chemotherapy, certain antibiotics, medicine for bowel disorders, medicine for high blood pressure, medicine to prevent organ transplant rejection, injectable osteoporosis medication, and some pain or arthritis medicines . Ask your pharmacist if you are concerned you are taking other medications that may harm your kidneys.
Other drugs may interact with valacyclovir, including prescription and over-the-counter medicines, vitamins, and herbal products. Tell each of your health care providers about all medicines you use now and any medicine you start or stop using.
What Is A Preferred Drug List
The Health First Colorado Preferred Drug List includes clinically effective medications that you can get without needing prior authorization or approval. This list is updated regularly. You may still be able to get drugs not on the Preferred Drug List. To get drugs that are not preferred on the Preferred Drug List, your provider will need to get prior authorization from Health First Colorado.
If you are already taking a drug not on the Preferred Drug List or your drugs gets removed from the Preferred Drug List, you may be able to keep taking the same drug. Talk to your provider about staying on your same drug.

How Should I Take Valtrex
Take Valtrex exactly as it was prescribed for you. Follow all directions on your prescription label. Do not take this medicine in larger or smaller amounts or for longer than recommended.
Start taking Valtrex as soon as possible after the first appearance of symptoms . This medicine might not be as effective if you first start taking it 1 or 2 days after the start of your symptoms.
Some herpes infections need to be treated for longer than others. Use this medicine for the full prescribed length of time. Your symptoms may improve before the infection is completely cleared. Skipping doses may increase the risk of your virus becoming resistant to antiviral medicine.
You may take Valtrex with or without food.
Tell your doctor if a child taking this medicine cannot swallow the tablet.
Drink plenty of water while you are taking Valtrex to keep your kidneys working properly.
Lesions caused by herpes viruses should be kept as clean and dry as possible. Wearing loose clothing may help to prevent irritation of the lesions.
Store Valtrex tablets at room temperature away from moisture and heat.
How To Use The Apple Health Pdl
Preferred drugs within a chosen therapeutic class are selected based on clinical evidence of safety, efficacy, and effectiveness. The drugs within a chosen therapeutic class are evaluated by the Drug Use Review Board, which makes recommendations to HCA regarding the selection of the preferred drugs.
The Apple Health PDL has products listed in groups by drug class. To obtain authorization for a nonpreferred drug, a client must have tried and failed, or is intolerant to, a designated number of preferred drugs within the drug class unless contraindicated or not clinically appropriate. The designated number of preferred drugs is listed on the Apple Health PDL.
In addition to approval criteria for nonpreferred products, some drugs have additional criteria that must be met. The requirement for additional criteria is noted in the columns on the Apple Health PDL.
Note: For continuity of care, all plans will use the same PA criteria, step therapy edits, and quantity limits developed by HCA.

What Is Valtrex
Valtrex is an antiviral drug. It slows the growth and spread of the herpes virus to help the body fight the infection.
Valtrex is used to treat infections caused by herpes viruses, including genital herpes, cold sores, and shingles in adults.
Valtrex is used to treat cold sores in children who are at least 12 years old, or chickenpox in children who are at least 2 years old.
Valtrex will not cure herpes and will not prevent you from spreading the virus to other people. However, this medicine can lessen the symptoms of an infection.
Valtrex may also be used for purposes not listed in this medication guide.
What To Avoid
Taking this medicine will not prevent you from passing genital herpes to other people. Herpes infections are contagious and you can infect other people even while you are taking Valtrex.
Avoid sexual intercourse or use a latex condom to help keep you from spreading the virus to others. Avoid letting infected areas come into contact with other people. Avoid touching an infected area and then touching your eyes. Wash your hands frequently to prevent the spread of infection.
Do not share this medicine with another person, even if they have the same symptoms you have.
