Medicaid Coverage Of Methadone Essential In Treatment Access For Opioid Use Disorders: Study

Critiquing the methadone clinic next door

February 11, 2016 by Alison Knopf

States that have Medicaid funding for methadone treatment are far more likely than states without such funding to have Medicaid-enrolled patients in treatment programs, researchers have found. In 17 states, however, Medicaid does not pay for methadone treatment.

For the study, Medicaid Coverage for Methadone Maintenance and Use of Opioid Agonist Therapy in Specialty Addiction Treatment, the researchers analyzed the 2012 Treatment Episode Data Set from the Substance Abuse and Mental Health Services Administration. They used a sample restricted to individuals who were covered by Medicaid and admitted to treatment primarily for opioid use disorders.

In states with Medicaid coverage for opioid agonist therapy, 45% of individuals enrolled in Medicaid used the opioid agonist therapy, compared to 30.1% of individuals enrolled in Medicaid in states with only Substance Abuse Prevention and Treatment block grant funding, and 17% of Medicaid-enrolled individuals in states with no public coverage.

Opioid agonist therapy is treatment with methadone or buprenorphinethe TEDS system does not distinguish between methadone and other opioid addiction treatment, but states do make this distinction. Both methadone and buprenorphine are effective, and buprenorphine is now covered in every state Medicaid program, but methadone is not. Furthermore, Medicaid funding for methadone treatment is already under fire in some states.

What Is Covered By Medicaid

Medicaid provides certain mandatory benefits for enrollees, meaning certain medically necessary treatments will always be funded by Medicaid regardless of where you live. For example, Medicaid will always fund inpatient and outpatient care in hospitals and certain home health services.

Coverage for some treatments is optional, and states can decide whether to fund them for Medicaid beneficiaries. For example, states can choose whether to cover dental treatment and prescription medications.

How Much Does Methadone Cost Without Insurance

While the cost of attending a methadone clinic is likely to vary from clinic to clinic, there is a general figure of what methadone costs without insurance.

An entire year of methadone maintenance therapy is estimated to cost the clinic $4,700, or about $12.87 per day .

On average, the cost of methadone for an individual without insurance is roughly $10-$15 dollars a day, or around $80-$100 per week.

Which type a methadone clinic a person seeks, or how the clinic is funded, will likely factor into the cost of methadone without insurance. It may be best for a person to call the clinic and figure out what the cost will be for an uninsured person.

Many clinics may also use a sliding scale for payment, which means they can adjust the cost of treatment based on a persons income.

While receiving MMT without insurance may be doable, its important to know if any government funded insurance options, like Medicaid or Medicare, cover the cost of methadone clinics.

Most Likely Reasons To Visit A Methadone Clinic

Methadone has been legal in the country since 1947. It is known to suppress drug withdrawal symptoms for a day. Sometimes, it works for up to 36 hours after administration, according to the National Drug Intelligence Centers Methadone Fast Facts.

There are actually a variety of circumstances in which a person might visit the clinic. The most obvious is to seek relief from daily struggles with opiate abuse. People can go to their appointment to fulfill the requirements of an ongoing drug treatment program. Its also possible for someone who falls back on a habit to seek methadone to manage cravings or withdrawal symptoms.

The first time people visit a methadone clinic, they usually dont receive medication. They generally go through a screening in which they talk about the nature of their problem to a counselor or nurse.

Patients are asked about their history, potentially given blood and/or urinalysis tests, and informed about the treatment program, its goals, and its guidelines. Drug use history, co-occurring disorders, and the impact of substance abuse on life are assessed via a clinical evaluation, while a medical evaluation reviews health history, current health, present medication requirements, and overall condition in regard to being given methadone.

Medicaid is one of the most common means of state funding for addiction treatment centers. Its the primary source through which patients with no or limited means get the help they need for their alcohol or drug dependency. Medicaid is an ideal option for those who need alcohol or drug treatment but dont have any private health insurance.

These programs generally cover detox as well as outpatient treatment centers. In limited cases, Medicaid may fund inpatient or residential treatment facilities, but outpatient coverage is more common due to the cost differential. Alternatives To Methadone Maintenance Therapy

While methadone maintenance therapy is a safe way to manage opioid dependency under medical supervision, there are effective alternatives.

One alternative is the medication Suboxone, which is buprenorphine combined with naloxone two drugs used to treat opioid dependence and addiction.

Suboxone reduces cravings and has a low risk of abuse. It comes in tablet form and is taken once every 24 hours. Suboxone is safer than methadone because a person cannot overdose from taking too much suboxone.

If a person in methadone treatment takes another opioid, they risk overdose, which can be fatal. Suboxone, however, contains naloxone, which is used specifically to reverse the effects of an opioid overdose.

Suboxone is likely given in an inpatient treatment center, or hospital, as a form of medication-assisted-treatment, or MAT.

MAT is an effective course of treatment to help with opioid withdrawal and dependence, and allows staff at an inpatient treatment center to closely monitor a persons progress and administer medication accordingly.

Inpatient facilities require the person to stay overnight, and they will likely receive around the clock, 24-hour care.

Many inpatient treatment centersare likely covered by private insurance plans, or even Medicaid, and dont require a person to travel everyday, sometimes for months or years, to receive their maintenance dose of methadone.

Government Programs: The Affordable Care Act

On March 23, 2010, President Obama signed the Affordable Care Act into law. The three main goals of the legislation were:

  • Making health insurance more affordable for more Americans through tax credits
  • Expand Medicaid coverage to all adults with an income below 138% of the federal poverty level
  • Lower health costs through new technology and methods
  • Many medical professionals praise the increase in availability of substance abuse treatment afforded by the ACA. Now considered an essential health benefit, insurers must cover services for the full spectrum of substance use disorders .

While rollbacks under the Trump administration have altered parts of the ACA, many core elements havent changed. In December 2017, Congress decommissioned the individual mandate, which required most to either enroll in health insurance or pay a penalty on their taxes. The open enrollment period for 2018 was shortened and funding for enrollment-assistance advertising was decreased. Still, some states have introduced their own healthcare laws in an effort to maintain stability should further cuts to the ACA be made.

    Does Private Insurance Cover Methadone

Private insurance may or may not cover the cost of methadone clinics. Common private insurance plans include HMO and PPO plans.

HMO, or Health Maintenance Organization plans give a person access to doctors and other healthcare options within a particular network. The network is usually made up of a group of providers that have agreed on lower rates with the same standard of coverage and quality.

PPO, or Preferred Provider Organization plans allow more flexibility for the doctors patients can see and usually cost a little more than HMO plans.

Interested in learning more about methadone and medication-assisted treatment programs?

Our treatment specialists are standing by to answer any questions you may have.

Whether or not an HMO or PPO plan covers the cost of MMT depends on a variety of factors. It can come down to whether or not methadone treatment is medically necessary, or whether methadone is a covered medication included in the plan.

Some plans may cover the cost of methadone, but still require a co-payment.

The coverage of methadone is likely to vary from plan to plan, but a person with a PPO plan may have more options than someone with a more traditional, and less expensive, HMO plan.

There may also be private methadone clinics that deal directly with insurance companies, and may cover part, or all, of methadone maintenance therapy.

It may be best to contact your insurance provider for more information.

Recognizing Opioid Addiction As A Medical Disorder

The stigmas attached to methadone clinics are quickly falling away as more scientific research shows that opiate addiction is not something that can easily be overcome through sheer willpower. It is a chronic and relapsing disease that results from physiological changes in how the brain functions and reacts to repeat use of opiates.

All opiates produce similar effects with varying degrees of potency, amounts, and methods of use and the individual has little to no control over developing a tolerance as the brain becomes adapted their presence. Once the person develops a dependency, cravings and physiological reinforcements are natural side effects that lead to addiction where almost any internal or external factor can be a trigger to use.
