Due to passage of State Question 802, Oklahomas Medicaid program was expanded starting last July to include many able-bodied adults. The expansion, which was authorized by the federal Affordable Care Act , was touted as a way to expand health services with little real cost to state governments because a high rate of federal matching funds was provided.

But during an Oklahoma Health Care Authority budget presentation to state lawmakers this week, OHCA officials listed sustainable funding for expansion population as one of the major challenges facing the agency.

We still have to work on a sustainable funding mechanism for expansion, Kevin Corbett, CEO of the Oklahoma Health Care Authority, told members of the House Appropriations and Budget Committee.

Last year, lawmakers had to provide an additional $164 million in state appropriations just for the Medicaid expansion population. That money came mostly from an increase in the Supplemental Hospital Offset Payment Program fee assessed on some hospitals revenue.

However, hospital officials have since announced they no longer want to pay the SHOPP fee, even though hospital officials previously endorsed both the creation of SHOPP and the expansion of Medicaid. Hospital officials instead urged lawmakers to divert other tax collections away from other uses.

The added costs created by expansion come amidst a dramatic surge in the overall cost of Oklahomas Medicaid program.

How Effective Is Medicaid

Medicaid is very effective in providing health insurance coverage to the most vulnerable. Since the ACAs major coverage expansions took effect in 2014, Medicaid has helped to reduce the number of uninsured from 45 million to 29 million. If Medicaid did not exist, most of the tens of millions of Medicaid enrollees would be uninsured. This is because private health insurance is generally not an option for Medicaid beneficiaries: many low-income workers do not have access to coverage for themselves and their families through their jobs and cannot afford to purchase coverage in the individual market. The creation of Medicaid, subsequent expansions of Medicaid coverage to children and pregnant women in the 1980s and 1990s, and the most recent expansion of Medicaid coverage to low-income adults under the ACA all have led to significant drops in the share of Americans without health insurance coverage.

Medicaid is also effective in improving access to care, in supporting financial stability among low-income families, and in improving health outcomes. Some of the clearest evidence comes from the ACA expansion of Medicaid coverage to low-income adults, which provides a recent natural experiment, letting researchers compare outcomes in states that did and did not adopt the expansion.

    New York Has Widened Its Lead In Per

    New Yorks per-capita Medicaid spending soared to more than double the nationwide rate in 2018, widening its gap with the other 49 states.

    New Yorks version of Medicaid, a health plan for the low-income and disabled, cost $3,869 per resident during the federal fiscal year ending in September 2018. That was 107 percent higher than the nationwide cost of $1,866 per resident.

    New Yorks rate was also 29 percent higher than the second-highest state, which was Alaska.

    New York was topped only by the District of Columbia, a city with a large low-income population, which spent more at $4,218 per person.

    This analysis is based on the most recently available Medicaid Financial Management Report from the federal Centers for Medicare & Medicaid Services.

    The disparities between New York and the rest of the country had widened significantly in just two years. In 2016, New Yorks per-capita spending of $3,236 was 79 percent higher than the national rate and 14 percent higher than the No. 2 state, which at the time was Vermont.

    Albanys spending surge came at a time of flat or declining overall enrollment, both in New York and nationwide.

    During that period, however, New York was experiencing a sharp increase in demand for home-based care, especially the non-medical services known as personal assistance.
