If you are filing a claim for an out-of-network provider for an emergency or because you were referred to the provider by UnitedHealth Group you must submit the following information
- The Policyholder’s name and address.
- The patient’s name and age.
- The number stated on your ID card.
- The name and address of the provider of the service.

- A diagnosis from the Physician.

RENO, Nev.—-Anthem Blue Cross and Blue Shield has partnered with Reno Doula Project to launch a new maternal health program in Washoe County that makes doula services available to the Medicaid health plans pregnant members at no cost. This program was designed in response to Nevadas poor health outcomes. According to Americas Health Rankings, Nevada ranks 46th of the 50 states when it comes to women and childrens health, mostly due to the states low physician-to-population ratios.

If you are moving from another state to Nevada, you most likely have heard of Blue Cross or Blue Shield before. In Nevada, this carrier is more commonly known as Anthem. Anthem is a national carrier that has contracted doctors and hospitals all over the United States. If you are looking for Blue Cross Nevada plan, Anthem is your closest option. Anthem provides a variety of different coverage options for most markets in Nevada. The two most common lines of coverage you might be getting with Anthem are employer group plans or Medicare.
Group Plans
Anthem offers employer sponsored plans for both small and large groups in Nevada. A small group is defined as any employer with at least 2 full-time employees but no more than 50. As long as you have 2 employees, that arent spouses, you are eligible for a group plan. Generally, if you have more than 50, you will be considered a large group. The plans and rates available for the small group market are vastly different from large group.
Medicare
Medicare Advantage plans usually have no monthly premiums to enroll. It also often covers prescription drugs and other expenses such as dental, vision, and hearing aids. In exchange, you will have higher out of pocket costs when using a Medicare Advantage plan. You will also need to use their specific doctors that are within the plans network which may limit you to certain providers.

What Procedures Are Covered By Anthem Bcbs Nevada
Major, proven bariatric surgical procedures such as the gastric sleeve and the gastric bypass are typically covered if the policy covers bariatric surgery in general. The duodenal switch, while less popular, may also be covered by the policy. The Lap-Band, or adjustable gastric band, may also be covered, but we typically do not recommend this to our patients as we have seen an unacceptably high long-term complication rate.
The gastric balloon is not covered by BCBS Nevada and any other procedure considered experimental or unproven is typically not covered by insurance.
Revisions to a previous bariatric procedure may be covered on a case by case basis. Most commonly, the removal of a Lap-Band and revision to a gastric sleeve is a covered procedure. It is important that you speak to us as well as your insurance company to understand the qualifying criteria for revisions

UnitedHealth Group was founded in 1977 by Richard Burke and went public in 1984. It was Dr. Paul Ellwood, who coined the term health maintenance organization, and who helped Burke start-up UnitedHealth. In 1979, the company created the first network senior health plan.
Blue Cross was formed in 1929 as a partnership between Baylor University Hospital and its patients who were struggling financially. Does Anthem Bcbs Nevada Have Pre
The short answer is yes. BCBS Nevada and all other insurance companies want to ensure that surgery is absolutely necessary before approving coverage. Therefore, they require documentation to that effect. Each plan may have different requirements, but in general, the following are guidelines stipulated by most insurance companies
Body mass index or BMI: a BMI of 35 or over with one or more comorbidities or obesity related diseases or a BMI of 40 or over regardless of comorbidity is necessary to qualify for bariatric surgery. These are guidelines set forth by the FDA. Comorbidities can include type two diabetes, high blood pressure, high cholesterol, sleep apnea and more
Morbid obesity: A diagnosis of morbid obesity must have been established for a certain amount of time prior to surgery. This can be up to three years. Your primary care physician will have to submit documentation to that effect. If no documentation has been established, timestamp photographs may be acceptable.
Medical weight loss: Also known as physician supervised weight loss, participation in a medical weight loss program is a likely prerequisite for surgery. This weight loss program may be required in the three or even six months leading up to surgery. During this time, patients will have to prove their participation and the treating physician will submit periodic evaluation to the insurance company.
Smoking: Patients must quit smoking at least two months before surgery.
